If mites or similar then he would be spending all his time scratching and rubbing....and the head around ears, eyes would be effected far greater

I do not believe that this is the case but maybe dease based like beck and feather.
Also what is of notice is the length of the claws... kakariki do have long claws but those are excessive and about now or sooner he should have bitten them off correct length.
I strongly suggest an avairian vet who should also take blood samples .

Im afraid over the3 long term I do not have much hope for the bird...
OK I put myself in your position.. what would I be doing?
1/ invermection.. drop on the back of the neck and repeat every 10 days 3 times.... leave for couple weeks and if no improvement repeat

2/Diet...see the thread "what do u feed..." cooked chicken bones and espec the cartilage from between the leg bones, and the marrow inside and the breast bone... chopped up into peas size.
This should be part of the regular diet anyway.

3/ mist Spray cages / birds once a week with 50/50/ soln vinegar and water.. doesnt matter what sort... then after a couple months once every 3 to 4 weeks
This should also be part of regular preventive maintenance anyway.

4/ Add a cap of apple cider vinegar to the veggie mix, feed at least once a week.. again should be regular maintenance .. may also add to the water to, once a week.

All of the above is not just for kakariki , but most caged birds.
